CHILDREN'S CHOICE/EPSDT

 

Children’s Choice

 

Providing a variety of services to developmentally disabled individuals in a manner that is efficient, caring, and addresses their individual needs in one of the highest goals of Louisiana Health and Rehabilitation Center’s Children’s Choice Program. These services are provided by a team of well trained professionals (Personal Care Attendants) who are sensitive to the needs of and can facilitate programming for a variety of individuals who meet the admission criteria.

  •  Care giving assistance and support

  • Case Management

  • Family Support

  • Center Based Respite

  • Environmental Accessibility Adaptations

  • Family Training

  • Medical coverage via the Medicaid Program

Our Goal

Children’s Choice Program main goal is to offer supplemental support to children with developmental disabilities who currently live at home with their families, or who will leave an institution to return home.

Eligibility Requirements

Financial eligibility must be determined for all applicants by the BHF Medicaid eligibility office. The following criteria must be met:

·     Income – must be less than 3 times the Supplemental Security Income (SSI) amount for the child (excluding consideration of parental income beginning the month of entry into the Louisiana Children’s Choice); and

·     Resources – less than SSI resource limit of $2,000 for a child (excluding consideration of parental resources beginning the month after the month of entry into the Louisiana Children’s Choice); and

·     Disability – meets Social Security Administration (SSA) definition of disability; and

·     Age – Birth up to 18 years of age, (children will be transferred to another appropriate MR/DD Waiver when they reach age 19 and they continue to meet all waiver eligibility criteria); and

·     Citizenship – U.S citizen or qualified alien; and

·     Residency – Resides in Louisiana; and

·     Enumeration – has or will apply for Social Security Number; and

·     Continuity of Stay – has received a waiver service 30 days or more.

·     Limited Eligibility – the child must meet all waiver eligibility criteria.  We offer Supplemental Support Services to Developmentally Disabled (MR/DD):

·     Children who currently live at home with their families, or legal guardians, or with foster families, or

·     Children who will leave an institution to return home.
